Enrollment Process for 2nd Year to 4th year students

INSTRUCTIONS FOR ENROLLMENT

2nd – 4th Year

IMPORTANT

There are two (2) Registration Form links available:

  • DL Registration Form – For students who qualified for the Dean’s List (DL) during the 2nd Semester, Academic Year 2025–2026.
  • Non-DL Registration Form – For students who are not included in the Dean’s List for the 2nd Semester, Academic Year 2025–2026.

Please make sure to select the correct Registration Form link. Choosing the wrong link may cause delays in the processing of your enrollment.

Step 1:
Fill out the Registration Form completely and attach your DULY SIGNED CLEARANCE.

Wait for the email containing your Assessment of Fees and Certificate of Registration (COR). Once received, print a copy.

 

Step 2:
Choose your preferred payment option:

Option 1 – Online Payment:
Scan the provided QR code and refer to your Assessment of Fees for the exact amount to be paid. Complete the payment and add a 10.00 service charge to the total amount.

Option 2 – Onsite Payment:
Proceed to the Cashier’s Office at the LGU Complex and present your Assessment of Fees to pay your enrollment fees.

Step 3:
After payment, follow the appropriate procedure:

For Online Payment:

  • Print a copy of your proof of payment or transaction history.
  • Write your complete name on the printed proof of payment.
  • Present it to the Cashier’s Office to claim your Official Receipt (OR). You may do this on or before the opening of classes.
  • The signing of your Certificate of Registration (COR) will be done during the opening of classes after presentation of the official receipt and submission of duly signed clearance.

For Onsite Payment:

  • After receiving your Official Receipt (OR), proceed to the Registrar’s Office for the signing of your Certificate of Registration (COR). Submit your duly signed clearance.

IMPORTANT REMINDER

For Onsite Enrollment:
Please bring the following documents:

  • Printed copy of your Assessment of Fees and Certificate of Registration (COR) and Duly signed Clearance
  • Official Receipt (OR)

For Online Enrollment:
Please bring the following documents on the first day of classes:

  • Printed copy of your Assessment of Fees and Certificate of Registration (COR)
  • Duly signed Clearance
  • Official Receipt (OR) or Proof of Payment
