For BS Criminology Applicants medical shall undergo the required medical examination and submit the following laboratory and diagnostic results upon enrollment:
1. Chest X-ray (must be within 6 months)
2. Complete Blood Count (CBC) with Blood Typing, must be taken within 1 month
3. Urinalysis (must be taken within 2 weeks)
4. Hepatitis B(HBsAg) Test (must be within 6 months)
5. Medical Clearance from the Attending Physician (for applicants with comorbidities or existing medical conditions)
 

Important Notes:

1. The Physical Medical Examination and Dental Examination shall be conducted at the Pilar Community College Clinic by the College Attending Physician and College Dentist.
2. Applicants must present the original laboratory and diagnostic examination results during their scheduled physical medical and dental examination (July 27-28, 2026).
3. Laboratory and diagnostic examinations may be obtained from Rural Health Unit of any licensed government or private hospital, clinic, or diagnostic center. If any required examination is not available at the Rural Health Unit, applicants may undergo the examination at a private medical facility at their own expense.
4. The Medical Certificate shall be issued only by the College Attending Physician after the review of all submitted laboratory and diagnostic results and the completion of the physical and dental examinations. The issuance of Medical Certificate shall be based on the applicant’s overall medical fitness for admission to the Bachelor of Science in Criminology program.
5. All medical documents, original laboratory and diagnostic results, and other related medical records shall be placed in a long brown envelope (non-plastic) and submitted to the Pilar Community College Clinic during the applicant’s scheduled medical examination.
***Failure to submit the complete medical requirements within the prescribed validity periods shall render the applicant ineligible to proceed to the next stage of admission process, including the interview, and shall delay or prevent official enrollment in the Bachelor of Science in Criminology program.
